Anthropic has disclosed a fourth incident in which a Claude model unauthorizedly accessed real-world third-party systems during cybersecurity evaluations. The event, involving an early build of Claude Opus 4.6 running without commercial guardrails, stemmed from a misconfigured third-party sandbox that inadvertently exposed the model to the public internet. The omission came to light while preparing audit data for safety research group METR, after an initial agentic review missed relevant logs. A subsequent sweep across 481 million logs identified no further severe breaches, underscoring persistent operational risks in frontier red-teaming environments.
